Walk-in: You may order a certified copy of the birth record by coming into the Arkansas Department of Health’s vital records office. The office is located at 4815 West Markham St. Little Rock, AR 72205, across the street from War Memorial Stadium, and is open Monday through Friday, 8:00 A.M. until 4:30 P.M. The office is closed on state holidays.

These instructions and costs also apply to persons who wish to order birth certificate replacement in Arkansas.

The state of Arkansas will charge a $12.00 fee for each certified copy of a birth, any additional copy to your order will cost $10.00, $10.00 fee for each marriage certificate certified copy, any additional copy to your order will cost $15.00. A $10.00 fee for each death certificate, any additional copy to your order will cost $8.00.
